The Recovery Care Navigator, through assessment, education, case management and utilization of community resources, will help develop and implement a comprehensive recovery plan with a focus on evidence-based substance use intervention, prevention, and treatment. The Recovery Care Navigator will work closely with community partners to encourage and empower individuals to establish themselves as productive members of their families and communities.
